Copyright Form

Authors whose manuscripts have been accepted for publication in the Journal of Applied Pharmaceutical Biology and Microbiology (JAPBM) must complete and submit the official Copyright and License Agreement Form before the article can be published.

Authors do not transfer ownership of copyright to JAPBM, the Faculty of Health Sciences, or Universitas Muhammadiyah Mataram.

By signing the form, the authors grant JAPBM a non-exclusive right of first publication and authorize the journal to publish, reproduce, distribute, display, communicate, archive, preserve, index, and make the article publicly available.

OPEN-ACCESS LICENSE

Articles published in JAPBM are distributed under the Creative Commons Attribution 4.0 International License (CC BY 4.0).

This license permits users to copy, distribute, adapt, transform, and build upon the published work for any purpose, including commercial purposes, provided that appropriate credit is given, a link to the license is provided, and any changes are indicated.

WHEN THE FORM IS REQUIRED

The completed form is normally required after the manuscript has been accepted and before final publication.

The editorial office may request the form earlier when clarification is required regarding:

• Authorship.
• Copyright ownership.
• Third-party material.
• Permissions.
• Previous publication.
• Ethical or legal responsibilities.

HOW TO COMPLETE THE FORM

Authors must:

1. Enter the manuscript title and OJS submission identification number.

2. Enter the names and information of all authors accurately.

3. Confirm the originality of the manuscript.

4. Confirm that all authors have approved the submitted and accepted versions.

5. Confirm that permission has been obtained for copyrighted tables, figures, photographs, instruments, or other third-party materials.

6. Confirm compliance with research ethics, applicable laws, and journal policies.

7. Declare funding, competing interests, data availability, and substantive use of artificial intelligence where applicable.

8. Sign and date the form according to the instructions provided.

SUBMISSION OF THE COMPLETED FORM

The completed and signed form should be uploaded through OJS as an Other Supporting Document or submitted through the workflow location specified by the editorial office.

Authors should not send sensitive personal documents through unofficial email addresses or third-party agents.

One complete Copyright and License Agreement Form is required for each accepted article.

AUTHOR WARRANTIES

By submitting the signed form, the authors confirm that:

• The manuscript is original.
• The manuscript does not unlawfully infringe the rights of another party.
• All authors meet the applicable authorship requirements.
• All authors have approved publication.
• The corresponding author is authorized to communicate with JAPBM.
• All necessary permissions have been obtained.
• The article complies with the JAPBM Publication Ethics and Malpractice Statement.
• The authors accept publication under the CC BY 4.0 license.

PUBLICATION REQUIREMENT

JAPBM may delay publication until the Copyright and License Agreement Form has been completed, signed, and verified by the editorial office. /

FORMULIR PERJANJIAN HAK CIPTA DAN LISENSI

Penulis yang naskahnya telah diterima untuk diterbitkan dalam Journal of Applied Pharmaceutical Biology and Microbiology (JAPBM) wajib mengisi dan menyerahkan Formulir Perjanjian Hak Cipta dan Lisensi resmi sebelum artikel dapat diterbitkan.

KEPEMILIKAN HAK CIPTA

Hak cipta atas artikel yang diterbitkan tetap dimiliki oleh penulis.

Penulis tidak mengalihkan kepemilikan hak cipta kepada JAPBM, Fakultas Ilmu Kesehatan, atau Universitas Muhammadiyah Mataram.

Dengan menandatangani formulir, penulis memberikan kepada JAPBM hak non-eksklusif untuk menerbitkan karya tersebut pertama kali serta mengizinkan jurnal menerbitkan, memperbanyak, mendistribusikan, menampilkan, mengomunikasikan, mengarsipkan, melestarikan, mengindeks, dan menyediakan artikel kepada publik.

LISENSI AKSES TERBUKA

Artikel yang diterbitkan dalam JAPBM didistribusikan berdasarkan Creative Commons Attribution 4.0 International License atau CC BY 4.0.

Lisensi tersebut mengizinkan pengguna menyalin, mendistribusikan, mengadaptasi, mengubah, dan mengembangkan karya untuk tujuan apa pun, termasuk tujuan komersial, dengan ketentuan atribusi diberikan secara tepat, tautan lisensi disediakan, dan setiap perubahan dijelaskan.

WAKTU PENYERAHAN FORMULIR

Formulir yang telah diisi pada umumnya wajib diserahkan setelah naskah diterima dan sebelum publikasi akhir.

Kantor editorial dapat meminta formulir lebih awal apabila diperlukan klarifikasi mengenai:

• Kepengarangan.
• Kepemilikan hak cipta.
• Materi pihak ketiga.
• Izin penggunaan.
• Publikasi sebelumnya.
• Tanggung jawab etik atau hukum.

CARA MENGISI FORMULIR

Penulis wajib:

1. Mengisi judul naskah dan nomor identifikasi pengiriman OJS.

2. Mengisi nama dan informasi seluruh penulis secara akurat.

3. Mengonfirmasi orisinalitas naskah.

4. Mengonfirmasi bahwa seluruh penulis telah menyetujui versi yang dikirimkan dan versi yang diterima.

5. Mengonfirmasi bahwa izin telah diperoleh untuk tabel, gambar, foto, instrumen, atau materi pihak ketiga yang dilindungi hak cipta.

6. Mengonfirmasi kepatuhan terhadap etika penelitian, peraturan yang berlaku, dan kebijakan jurnal.

7. Mengungkapkan pendanaan, konflik kepentingan, ketersediaan data, serta penggunaan substantif kecerdasan artifisial apabila relevan.

8. Menandatangani dan mencantumkan tanggal sesuai petunjuk dalam formulir.

PENYERAHAN FORMULIR YANG TELAH DIISI

Formulir yang telah diisi dan ditandatangani harus diunggah melalui OJS sebagai Other Supporting Document atau melalui bagian alur kerja yang ditentukan oleh kantor editorial.

Penulis tidak disarankan mengirimkan dokumen pribadi melalui email tidak resmi atau agen pihak ketiga.

Satu Formulir Perjanjian Hak Cipta dan Lisensi yang lengkap diperlukan untuk setiap artikel yang diterima.

JAMINAN PENULIS

Dengan menyerahkan formulir yang telah ditandatangani, penulis menyatakan bahwa:

• Naskah bersifat asli.
• Naskah tidak melanggar hak pihak lain secara tidak sah.
• Seluruh penulis memenuhi persyaratan kepengarangan.
• Seluruh penulis telah menyetujui publikasi.
• Penulis korespondensi berwenang berkomunikasi dengan JAPBM.
• Seluruh izin yang diperlukan telah diperoleh.
• Artikel mematuhi Pernyataan Etika Publikasi dan Malpraktik JAPBM.
• Penulis menyetujui publikasi berdasarkan lisensi CC BY 4.0.

PERSYARATAN PUBLIKASI

JAPBM dapat menunda publikasi sampai Formulir Perjanjian Hak Cipta dan Lisensi telah diisi, ditandatangani, dan diverifikasi oleh kantor editorial.
