Author's Checklist Form
The Authors’ Checklist Form is a supporting document used by authors to ensure that their manuscript has fulfilled all submission requirements of Lumbung Pengabdian Kesehatan (JLP) before being submitted through the OJS 3 system.
This form helps authors check the completeness of the manuscript, including manuscript structure, article format, abstract, keywords, tables, figures, references, ethical statements, authorship information, and other required submission components.
By completing this checklist, authors confirm that the manuscript has been prepared according to the official JLP Article Template, Author Guidelines, Submission Preparation Checklist, and Publication Ethics. The form is also useful for the editorial office during the initial screening process to assess whether the manuscript is ready to proceed to the review stage.
Authors are encouraged to complete and upload the signed Authors’ Checklist Form as a supplementary file during manuscript submission.

Authors’ Checklist Form atau Formulir Checklist Penulis merupakan dokumen pendukung yang digunakan oleh penulis untuk memastikan bahwa naskah telah memenuhi seluruh persyaratan pengiriman naskah pada Lumbung Pengabdian Kesehatan (JLP) sebelum dikirimkan melalui sistem OJS 3.
Formulir ini membantu penulis memeriksa kelengkapan naskah, meliputi struktur artikel, format naskah, abstrak, kata kunci, tabel, gambar, daftar pustaka, pernyataan etika, informasi kepengarangan, serta komponen lain yang diperlukan dalam proses submission.
Dengan mengisi checklist ini, penulis menyatakan bahwa naskah telah disusun sesuai dengan Template Artikel resmi JLP, Author Guidelines, Submission Preparation Checklist, dan Publication Ethics. Formulir ini juga membantu editorial office dalam proses pemeriksaan awal untuk menilai apakah naskah telah layak dilanjutkan ke tahap review.
Penulis disarankan untuk mengisi dan mengunggah Authors’ Checklist Form yang telah ditandatangani sebagai file tambahan pada saat melakukan submission naskah.
Instructions / Petunjuk Pengisian
1. Download the Authors’ Checklist Form provided by Lumbung Pengabdian Kesehatan (JLP). / Unduh Authors’ Checklist Form yang disediakan oleh Lumbung Pengabdian Kesehatan (JLP).
2. Fill in the manuscript identity, including article title, author names, affiliation, and corresponding author information. / Isi identitas naskah, meliputi judul artikel, nama penulis, afiliasi, dan informasi penulis korespondensi.
3. Read each checklist item carefully before marking the appropriate column. / Baca setiap butir checklist dengan cermat sebelum memberi tanda pada kolom yang sesuai.
4. Ensure that the manuscript follows the official JLP Article Template and Author Guidelines. / Pastikan naskah telah mengikuti Template Artikel resmi JLP dan Author Guidelines.
5. Check the completeness of all manuscript sections, including title, abstract, keywords, introduction, methods, results and discussion, conclusion, acknowledgments, and references. / Periksa kelengkapan seluruh bagian naskah, meliputi judul, abstrak, kata kunci, pendahuluan, metode, hasil dan pembahasan, simpulan, ucapan terima kasih, dan daftar pustaka.
6. Ensure that all tables, figures, documentation photos, and references are properly written and placed in the manuscript. / Pastikan seluruh tabel, gambar, foto dokumentasi, dan referensi telah ditulis dan ditempatkan dengan benar dalam naskah.
7. Confirm that the manuscript complies with publication ethics, originality, authorship, and plagiarism requirements. / Pastikan naskah telah memenuhi ketentuan etika publikasi, orisinalitas, kepengarangan, dan bebas dari plagiarisme.
8. Sign the form after all checklist items have been reviewed. / Tandatangani formulir setelah seluruh butir checklist diperiksa.
9. Upload the completed and signed form as a supplementary file in the OJS 3 submission system. / Unggah formulir yang telah diisi dan ditandatangani sebagai file tambahan pada sistem submission OJS 3.
